Breast Cancer Patient Navigator
The Breast Cancer Patient Navigator (BCPN) guides breast cancer patients with a suspicious finding, such as cancer, through the healthcare system to ensure timely diagnosis, treatment and survivorship care.
Kelly White MSN, APN, ANP-BC brings over 10 years of oncology nursing experience to Shaw. Her goal is to facilitate an appointment within one week of a suspicious finding or the following Tuesday after a finding, during which time she gathers pertinent clinical data (such as scans and pathology) for review by Shaw's multi-disciplinary team including, but not limited to: breast surgeons, medical oncologists and radiation oncologists. Kelly then coordinates appropriate appointments for the weekly Tuesday Breast Clinic. After appointments, she provides education on the designated treatment plan, coordinates follow-up appointments and insures the patient has appropriate support resources for moving forward, including emotional support and nutritional guidance.
Upon completion of treatment, Kelly provides the patient with a survivorship care plan that outlines cancer history and treatment as well as anticipatory guidance for future preventative healthcare needs. She acts as a single point of contact throughout a patient's cancer care and maintains an ongoing dialogue with her patients to facilitate improved communication amongst their providers in a timely manner. Ultimately, the goal of the patient navigator is to decrease anxiety and fear associated with the cancer journey and facilitate smooth transitions throughout care.
