After many years of service, Vail Valley OB/GYN
closed its doors on December 19, 2011.Both Dr. Keith Samuels and Kerrie Kuhl have
remained in the Vail Valley to offer continuing medical care to existing and new patients. Dr. Samuels has joined Colorado Mountain Medical at the Edwards office (970-926-6340), and Kerrie Kuhl has moved to Vail Valley Medical Center’s Eagle Care Clinic in Edwards (970-569-7520). TO OBTAIN A COPY OF YOUR VAIL VALLEY MEDICAL CENTER MEDICAL RECORDS:
To begin a request for medical records, please download, print, complete and sign the following form. Please complete ALL portions of the authorization and bring it with you to the Health Information Management Department in Gypsum. To expedite the process, you may fax the completed form and a copy of the patient’s valid photo ID to 970-470-6641. Please specify if you would like to pick up the copies in person or have them mailed to you. We are unable to fax copies of patient medical records. Medical records are available within 10-14 days upon receipt of request for medical records. If you have any questions, please contact us at 970-777-2900.
