Hospice
The goal of Mountain Hospice is to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, in their own home surrounded by their loved ones. Hospice affirms life and does not hasten or postpone death. Hospice care treats the person rather than the disease, it focuses on quality rather than length of life. Hospice care is suitable when you no longer benefit from active treatment for the disease and are expected to live 6 months or less. Mountain Hospice care is a family-centered approach that includes a team of doctors, nurses, home health aides, social workers, chaplains, physical therapists, occupational therapists, and trained volunteers. They work together focusing on the dying person’s needs’ physical, emotional, or spiritual. The goal is to help keep the person as pain-free as possible, with loved ones nearby until death. The Mountain Hospice team comes to the home and develops a care plan that meets each person’s individual needs for pain management and symptom control. Call us at 970-569-7455 if you have questions. Our office is open from 8 am- 5:00 pm, Monday through Friday. Skilled Nurses are on-call 24 hours a day.
